In the first edition decompression of the carpal tunnel and release of trigger finger were included, but because of the risks of intravenous regional block anaesthesia they were omitted from the second edition. They have been reintroduced, together with stab avulsion of varicose veins and excision of ganglion, in a separate section on "Advanced minor surgery" and, instead of intravenous regional block, a much safer technique is described.
